We are looking for a dedicated to candidate join our team as a Lead Community Health Worker (CHW) to service our patients in the Brunswick region. A CHW has a particularly good understanding of the community served, acting as a liaison between health and social services and the community to facilitate access to services and to improve the quality and cultural competence of service delivery.
Lead Community Health worker supervises the Community Health workers ensuring patients are supported as well as performing visits in homes and facilities (ALF and ILF's). This is a full time, salary-based role working 8-hour shifts (8:00AM-5:00PM) Monday-Friday.

Workers (CHWs) within their designated region. This role ensures seamless service delivery,
compliance with care plans, and effective communication between patients, healthcare providers,
social services, and the community. The Lead CHW collaborates closely with care teams within the
organization to oversee and enhance patient care within their assigned region.
